A deadly bacterial outbreak is spreading through one of New York City’s most densely populated neighborhoods, and investigators still haven’t found the source.

Within days, the case count jumped — 14 confirmed by July 4, 23 by July 6, 46 shortly after, and then 60 cases by mid-July. Fifteen people were hospitalized at one point during the surge. Three people died. The speed of the escalation caught even seasoned health officials off guard.
Legionnaires’ disease is a serious form of bacterial pneumonia. People get sick by breathing in tiny water droplets containing Legionella bacteria — not by drinking contaminated water.
Symptoms show up two to fourteen days after exposure and include fever, cough, and trouble breathing. Older adults, smokers, and people with weakened immune systems face the highest risk of dying from it. That matters a lot on the Upper East Side, which has a large senior population.
Dozens of Buildings Flagged, but No Single Source Named
Health investigators zeroed in on rooftop cooling towers — the large water-based systems that cool buildings in summer heat. These towers can spray Legionella-laced mist into the air, where people nearby breathe it in.
By mid-July, 76 buildings had tested positive for Legionella bacteria in preliminary testing, including the Metropolitan Museum of Art.
Of the first 31 buildings identified, 19 had already drained, cleaned, and disinfected their cooling towers. Officials tested roughly 87 percent of the approximately 150 rooftop cooling towers in the affected area, yet still had not pinpointed a single definitive source.

Mayor Zohran Kwame Mamdani announced emergency measures on July 6, as the case count hit 23. The city expanded its investigation zone to include ZIP code 10075 after at least one case was linked to that area. Town halls were held for worried residents.
City Council Speaker Adrienne Adams and other officials demanded faster answers. The political pressure was real — and justified. When people are dying and the source is unknown, the public deserves urgency and transparency in equal measure.
New York City Has Been Here Before — and It Took Weeks to Find the Answer
This outbreak follows a grim pattern. In the summer of 2025, a Legionnaires’ cluster in Central Harlem grew to 118 confirmed cases and seven deaths before investigators traced it to two city-owned buildings — Harlem Hospital and the site of a new public health laboratory.
Before that, the 2015 South Bronx outbreak sickened dozens before whole-genome sequencing finally pointed to a single hotel cooling tower as the source.
Across multiple outbreaks since 2015, New York City has recorded 213 cases and 18 deaths tied to cooling towers. Each time, it took weeks of lab work to find the culprit.
That history should sharpen the city’s response — and its sense of urgency. The good news in the current outbreak is that new diagnoses began slowing by mid-July, suggesting the worst of the exposure may have passed. But “slowing” is not “stopped,” and three deaths have already occurred.
The fact that the source remains unconfirmed weeks into the investigation raises a fair question: are the city’s cooling tower inspection and maintenance rules tough enough, or are they being enforced consistently? The 2025 Harlem outbreak showed that even city-owned buildings can be the problem. That is not a reassuring track record.
What to Watch for and What to Do
Health officials urge anyone who lives, works, or has recently visited the Upper East Side to watch for symptoms — fever, cough, muscle aches, or shortness of breath. See a doctor right away if symptoms appear. Legionnaires’ disease is treatable with antibiotics when caught early.
The longer someone waits, the worse the outcome can be. Residents near buildings that tested positive have been advised to keep windows closed and limit outdoor exposure until the source is confirmed and cleaned.
Investigators are still working to match specific bacterial strains from patients to specific building water systems. That lab work — the same kind used in 2015 and 2025 — takes time.
Until a match is confirmed, the full scope of who is responsible remains open. What is not open is the basic obligation: New York City must find the source, fix it, and explain how it was allowed to become a threat in the first place.
